DATA ELEMENT DICTIONARY - CM&E REPORTING TABLES

The data fields for the reporting tables associated with the CM&E Module are documented, by table, below.   

 

CE_REPORTING

English Name Description
EPA Handler ID

Unique RCRA identification number assigned by the implementing State or Region to each RCRA site (e.g., generators; transporters; and treatment, storage, disposal facilities).  Note: Handlers associated with Navajo Nation should be assigned Handler IDs beginning with 'NN' regardless of where the handler is physically located.

Evaluation Activity Location

Indicates the location of the agency conducting the evaluation. [see nationally-defined values]

Evaluation Identifier Name or number assigned by the implementing agency to identify an evaluation.
Evaluation Start Date The first day of the inspection or record review regardless of the duration of the inspection.
Evaluation Responsible Agency Indicates the agency responsible for conducting the evaluation.  Note: There is no relationship between a State's authorization status and the Evaluation Responsible Agency. The value 'L' - Local is only available to Region 9 states unless requested by other states/regions.
Found Violation

Indicates whether an evaluation discovered any violations at the facility. For CAV evaluations, the found violation indicator shows whether the evaluation found any discrepancies at the facility. [Y - Yes; N - No; U - Undetermined]

Citizen Complaint Indicator

Indicates whether the evaluation was initiated because of a tip/complaint. [Y - Yes; N - No]

Multimedia Indicator

Indicates whether the evaluation was part of a multimedia inspection activity. [Y - Yes; N - No]

Sampling Indicator Indicates whether the evaluation conducted included the collection of samples. [Y - Yes; N - No]
Not Subtitle C Indicator For an inspection that was conducted pursuant to RCRA 3007 or State equivalent, indicates whether the site is not a Subtitle C facility. [Y - Yes; N - No]
Evaluation Type Code indicating the type of evaluation conducted at the facility. [see nationally-defined values]
Evaluation Type Description English description of the type of evaluation.  
Focus Area Code identifying the subject of a Focused Compliance Inspection.  [see nationally-defined values]
Focus Area Description English description of the focus area.  
Evaluation Responsible Person Identifier Code indicating the person within the agency that is responsible for conducting the evaluation.
Evaluation Responsible Suborganization Code indicating the branch/district within the agency that is responsible for conducting the evaluation.
Handler Activity Location Indicates the location of the agency regulating the handler.  [see nationally-defined values]
Handler Name The legal name of the site.
Region The two-digit code for the region in which the handler is located.   [see nationally-defined values]
State State postal code for state in which handler is located.  [see nationally-defined values]
Land Type Code indicating current ownership status of the land on which the facility is located.  [see nationally-defined values]
Request Sequence System-generated sequence number for each request record.
Date of Request The date that a 3007 information request was submitted.
Date Response Received The date that 3007 information is received.
Request Agency Code indicating the agency responsible for submitting the 3007 information request.  [S-State; E-EPA]
Request Activity Location Indicates the location of the agency requesting the 3007 information.  [see nationally-defined values]
Violation Activity Location Indicates the location of the agency regulating the activity.  [see nationally-defined values]
Violation Sequence Number System-generated sequence number for each violation record.
Agency Which Determined Violation The agency that actually determined the violation.   [S-State; E-EPA]
Violation Type

Code indicating the specific area of the RCRA portions of the Federal Code of Regulations covered by the evaluation and/or found to be in violation with RCRA regulations or statutes. Areas generally correspond to subparts of CFR 40: Part 262 for generators, 263 for transporters, and 264/265 for TSDs. Note: Implementers who are broader-in-scope will be able to use Violation Type 'XXS'. Additional clarification should be addressed through the use of the Citation field. Violation Short Description

English description (short version) of the violation. 

Former Citation

The citation from Federal or State regulations or statutes that specifies a violation or pending violation in sufficient detail to distinguish it from other violations intended to be addressed separately by other enforcement actions. These regulation numbers or citations can be found in the Code of Federal Regulations or in State regulation publications.  Note: This field is provided to show historical information, however, this field is no longer used.

Date Violation Was Determined

The date that a determination is made that the violation exists. This is not necessarily the same date as the date of the inspection or evaluation.  For example, the Date Violation Was Determined may be when the agency receives sample results or a legal determination.

Actual Return to Compliance Date

The date the agency determines that the handler demonstrated full physical compliance (the date compliance was verified).  The handler will be considered to be out-of-full-physical-compliance until the Actual Returned to Compliance Date has been determined.  Actual Return to Compliance Date does not necessarily mean that all enforcement actions are completed for this violation. For violations of omission (such as not manifesting a load of waste), the Actual Return to Compliance Date is the date of a written commitment by the handler to comply in the future or the day of conviction in a criminal action.  Penalty payment is not a condition of full physical compliance; however if non-payment is the only violation then the Actual Return to Compliance Date is the date that payment is received.

Return to Compliance Qualifier

Code indicating the conditions under which actual compliance was observed.  Note:  The value "O" (observed) was used for ALL RTC violations brought forward from RCRIS.

Violation Responsible Agency

Code indicating the agency responsible for the violation.

Scheduled Compliance Date

The date by which the handler is to submit to the agency its documentation that the violation has been brought into compliance.  Scheduled response dates are specified in enforcement actions as the compliance schedule.  If a number of activities are to be performed according to a compliance schedule with more than one date, this is the date of the last action to be taken by the handler to return to compliance.

Enforcement Activity Location

Indicates the location of the agency regulating the activity.  [see nationally-defined values]

Enforcement Identifier

Name or number assigned by the implementing agency to identify an enforcement.

Date of Enforcement Action

The date the enforcement action was issued. For all formal actions involving written documents, this date is the date the document is signed.  For an informal action, this date is the date the handler received actual notification.

Enforcement Responsible Agency

Agency responsible for issuing the enforcement action.  [S-State; E-EPA]

Enforcement Docket Number

The relevant docket number that enforcement staff have assigned for tracking of enforcement actions.

Enforcement Attorney

Identifies the attorney within the agency that is responsible for issuing the enforcement action.

Corrective Action Component

Indicates whether the enforcement action contains a corrective action component.  [Y - Yes; N - No]

Appeal Initiated Date

The date that the appeal was initiated.

Appeal Resolution Date

The date the appeal was resolved.

Disposition Status Date

The date that the enforcement action was closed, terminated, or discontinued.

Disposition Status

Code indicating how an enforcement action was closed, terminated, or discontinued.  [see nationally-defined values]

Disposition Status Description

English description of the disposition status. 

Consent Agreement/Final Order (CA/FO) Sequence Number

Identifies the consent agreement / final order that this enforcement action is a part of.

CA/FO Respondent Name

Name of the respondent associated with the CA/FO.

CA/FO Lead Agency

The lead agency that is responsible for defining the CA/FO.  [see nationally-defined values]

Enforcement Type

Code identifying the type of action being taken against a site.  [see nationally-defined values]

Enforcement Type Description

English description of the enforcement type. 

Enforcement Responsible Person

Code indicating the person within the agency responsible for the enforcement action.

Enforcement Responsible Suborganization

Code indicating the branch/district within the agency that is responsible for the enforcement action.

SEP Sequence Number

A system-generated sequence number to uniquely identify Supplemental Environmental Projects for a given site.

SEP Expenditure Amount

The actual amount of the Supplemental Environmental Project expenditure.

SEP Scheduled Completion Date

The date set by an enforcement action for the full or partial expenditure of funds for a Supplemental Environmental Project.

SEP Actual Date

The actual date of a Supplemental Environmental Project expenditure.

SEP Defaulted Date

The date a site defaulted on a Supplemental Environmental Project.

Supplemental Environmental Project (SEP) Type

Code indicating the type of Supplemental Environmental Project associated with the enforcement action. [see nationally-defined values]

SEP Type Description

English description of the Supplemental Environmental Project type. 

Proposed Amount

The proposed penalty assessed by the enforcement action.

Final Monetary Amount

The final monetary penalty assessed by the enforcement action.

Paid Amount

The total amount paid to date by the handler.  Note:  This information is not required to be entered by the implementing agency.

Final Count

The number of final monetary penalties assessed against this handler.

Final Amount

The total penalties (final monetary penalty + Supplemental Environmental Project credit) assessed against this handler.  

Notice of Compliance Date

Date that the facility was notified that there were no violations discovered or that the violations previously discovered by this evaluation have returned to compliance.

Financial Assurance Requirement

Indicates whether the enforcement action contains a financial assurance requirement.  [Y - Yes; N - No]
